The Role


The LPN shall serve as a member of the medical team and will provide direct patient care under the supervision of a Registered Nurse or Medical Provider and within the LPN scope of practice. This work is carried out in support of the mission and philosophy of Samaritan Daytop Health, Inc.

Responsibilities


What You Will Do


  • Demonstrates initiative, competency and enthusiasm for carrying out daily assignments; assisting and taking direction from other health care providers in the delivery of patient care and maintaining patient flow.
  • Accurately collects clinical data on patients and demonstrates competency in performing clinical interventions.
  • Promptly reports identified abnormalities in clinical data to the Medical Provider or R.N. on duty.
  • Accurately documents nursing observations, interventions and patients’ response to treatments in the
  • patient’s EHR in accordance with established standards, policies and procedures.
  • Demonstrates a commitment to maintaining a supportive work environment that fosters collaboration and communication between all health care team members, projects a positive image to the communities we serve and demonstrates respect for the cultural and religious diversities and needs of patients, families and care providers.
  • Demonstrates the ability to identify and set priorities in the provision of care.
  • Utilizes the Health Information Technology to perform essential job functions, which include but are not limited to (1) Documentation in the Electronic Health Record (EHR), (2) Transmission of Electronic Health Information (EHI) (3) Retrieval of Electronic Health Information (EHI).
  • Organizes and maintains the integrity of the EHR in accordance with agency policies and procedures.
  • Facilitates communication with appropriate staff in the therapeutic communities regarding urgent treatment plans in accordance with agency procedure.

Qualifications


Who You Will Be


  • High School Diploma or GED.
  • Graduate of accredited program of Practical Nursing,
  • At least Three (3) years in the ambulatory care setting.
  • Computer literacy including proficiency in Microsoft Office Suite and EMR.
  • Excellent interpersonal, communication, and organizational skills.
  • Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.
