The Role


Under the general direction of the Program Director, the Assistant Director is responsible for providing ongoing support to the Program Director, managing the day-to-day operations and service delivery, and participating in activities to monitor the quality and appropriateness of program services. Additionally, the Assistant Program Director will coordinate coverage for the facility, provides on-call administration as needed, and manages facility emergencies. Hope Care Management provides extensive, well-coordinated case management services for HIV/AIDS. Comprehensive case management includes case management services to family members and significant others who may have an impact on the lives of the identified client. In these programs, participants receive a full, comprehensive bio-psychosocial needs assessment, an extensive service plan, and coordinated linkage to all services required to help them and their families live healthy, productive lives. This work is carried out in support of the mission and goals of Samaritan Daytop Village.

Responsibilities


What You Will Do


  • Coordinates and implements case management services for an assigned multidisciplinary treatment delivery team.
  • Helps implement and monitor policies, procedures, and systems necessary for the effective and efficient delivery of program services.
  • Provides regularly scheduled administrative supervision to assigned staff.
  • Documents supervisory sessions with staff.
  • Helps identify staff’s training needs and, in response, plans, develops, and/or conducts training toward maintaining and improving staff competency and compliance with agency policy and procedure.
  • Ensures the maintenance of accurate, complete, and timely client treatment records by staff that complies with regulatory requirements and agency internal policy and procedure.
  • Adheres to all responsibilities and duties of a New York State-mandated reporter.
  • Ensures that all program staff upholds the agency's Code of Conduct/Code of Ethics and complies with all Federal, state, and local laws/regulations, including 42 CFR confidentiality and HIPAA privacy and security regulations.
  • Networks and communicates with other departments as a member of the program’s management team.

Qualifications


Who You Will Be


  • Master’s Degree in Social Work or related field and at least One (1) year of experience in a Health Home program and/or Care Coordination program.
  • Minimum Three (3) years supervisory experience in a behavioral health setting.
  • General understanding of chronic conditions i.e. HIV, Substance Abuse and Mental Illness Disorders, and Care Coordination.
  • Flexibility is needed as members may call outside of their daily work schedule (24-hour call) and/or work a flexible schedule as needed in response to program and staff needs.
  • Computer literacy including proficiency in Microsoft Office Suite and EHR.
  • Ability to work under pressure with excellent organizational skills.
  • Ability to maintain the confidentiality of patient records.
  • Experience working directly with people from diverse racial, ethnic, and socioeconomic backgrounds.
