The Role


Hope Care Management provides extensive, well-coordinated case management services to HIV/AIDS. Comprehensive case management includes case management services to family members and significant others who may have an impact on the lives of the identified client. In these programs, participants receive a full, comprehensive bio-psychosocial needs assessment, an extensive service plan, and coordinated linkage to all services required to help them and their families live healthy, productive lives.

 Under general direction of the Program Director, The Outreach Coordinator and Systems Manager (OCSM) will collect, analyze, monitor and report internal and external (DOH, Lead Health Homes, UAS) data to the Hope Care Management Health Homes program. The OCSM will effectively combine diverse data sets and report the findings to the Director of Hope Care Management and Executive Leadership. The OCSM provides guidance, supervision, and administrative oversight to the outreach and intake staff at Hope Care Management. This work is carried out in support of the mission and goals of Samaritan Daytop Village.

Responsibilities


What You Will Do


  • Analyze data and processes to determine changes to existing reports or new reporting needs that will support program objectives and provide insight to the effectiveness of program initiatives.
  • Creates user accounts for (but not limited to), PACT, GSI, RMA, Epaces, Health Commerce System (HCS).
  • Conducts PC-MVS reports bi-weekly to distribute to Care Coordination Supervisors, HARP Specialists and the Program Director.
  • Evaluates encounter data and identifies errors created by operational, claims and eligibility processes.
  • Collects and distributes utilization data to the Care Coordination Supervisors to address client needs.
  • Coordinates with the Lead Health Homes and Managed Care Organizations to address ongoing segment issues, and enhance program outcomes.
  • Reviews and evaluates discrepancies within reports created by the Lead Health Homes and internal data; reports all issues to Program Director, promptly.
  • Attends department, DOH, Lead Health Homes and meeting as needed/requested.
  • Collaborates with other members of the Hope Care Management team to identify and define programmatic challenges and primary objectives.
  • Provides training to all new staff members as needed.

Qualifications


Who You Will Be


  • BA in Accounting, Finance, Business Administration or related field.
  • At least Two-Three (2- 3) years' experience in billing.
  • Prior work experience in Health Home or Care Coordination program.
  • Computer literacy including proficiency in Microsoft Office Suite and EHR.
  • Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.
  • Excellent interpersonal, organizational skills, analytical and problem-solving skills.
  • Attention to detail and high level of accuracy.
  • Knowledge of office administration and billing procedures.
  • Ability to work independently with little supervision.
  • Excellent critical/analytical thinking skills.
  • Able to identify and resolve issues in a timely manner.
  • Adaptable to change in work environment.
  • Ability to maintain a high level of accuracy in preparing and entering client data.
  • Maintain a high-level confidentiality concerning client information.
  • Evidence of excellent interpersonal skills; ability to supervise, coach, mentor, develop, train and teach staff.
