The Role

Under the general direction of the Program Director, the Targeted Case Manager (TCM) is responsible for supporting the wellness and recovery goals by implementing targeted interventions designed to provide timely, high quality and efficiency care. The TCM is to provide access to services that would encourage clients to resolve problems that interfere with their independence or self-sufficiency as well as maintain themselves in the community.

Responsibilities

What You Will Do

  • Provides case management to Clients who are Health Home eligible, adults diagnosed with severe mental illness, adults with two services in an inpatient, outpatient, residential and detoxification substance use disorder (SUD) program within a year. The TCM provides services to adults incarcerated for more than 30 consecutive days in the past twelve months.
  • Provides individual counseling and facilitation of caseload group meetings and other assigned groups.
  • Provides support for improved community service linkages and referral to the community and social support services.
  • Provides transitional care, support in navigating complex health care and social services system.
  • Provides individual and family support for the clients and advocates for needed services.
  • Performs on-site crisis intervention and skills teaching when other services are not provided.
  • Monitors that the services in the treatment plan are adequately addressing individual’s needs and wellness and recovery goals.
  • Monitoring and follow-up activities include contact with the service recipient, the recipient’s family and collaterals, service providers, and other individuals or entities.

Qualifications

Who You Will Be

  • Bachelor’s Degree in Human Services required.
  • Masters in Social Work preferred.
  • Bilingual Spanish (preferred).
  • Minimum 2+ years of experience in counseling severe clients with substance use disorders and/or mental illness.
