The Role


Under general direction, the Outreach Coordinator is responsible for providing quality outreach initiatives within specified catchment areas and generating referrals for individuals seeking treatment to SDV continuum of services. Activities also involves participating in departmental committees, staff meetings, community groups and /or advisory councils; accepting public speaking requests regarding program services and/or available community services.

Responsibilities


What You Will Do


  • Conduct presentations on SDV services to referral agencies in the community.
  • Conduct interviews in settings like hospitals, prisons, jails, and in the courts.
  • Conduct admission interviews in accordance with best practices.
  • Review admissions documentation with QHP staff or supervisor to determine eligibility for treatment through clinical assessments.
  • Establish and maintain contact with referral agencies.
  • Participate in weekly inter-disciplinary team meetings to assess the needs of the clients and assist in facilitating admissions.
  • Help track and monitor admission trends, including veterans, criminal justice referred, and other specialty populations.
  • Coordinate pick up of new admissions at referral agencies.
  • Provide charting and documenting of services provided.

Qualifications


Who You Will Be


  • High School Diploma or Equivalent and hold a CRPA or CASAC-T certification.
  • Minimum of two years of work experience.
  • Knowledge of substance use disorder recovery process.
  • Computer literacy including familiarity with Microsoft Office Suite.
  • Must be available to travel throughout the required catchment area as needed.
  • Commitment to recovery-oriented systems of care.
  • Demonstrate ability to work independently.
  • Excellent time management Skills
  • Ability to think logically, prioritize, and follow procedures and instructions
  • Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.
  • Experience with advocacy, outreach and engagement, self-help, recovery, and transitional supports.
