The Role


The SCN Supervisor/Coordinator will be responsible for overseeing the implementation of the SCN framework within Samaritan Daytop Village, ensuring alignment with NYC’s broader social care objectives. The SCN Supervisor/Coordinator position entails several elements of responsibilities to ensure the care management component of Samaritan Daytop Village Inc. is satisfactorily meeting the core values of care management services as set forth in the federal guidelines of the Affordability Care Act. The SCN Supervisor/Coordinator will serve as a conduit between external and internal entities to facilitate communication between all parties to ensure effective outcomes in the provision of care management services. The SCN Supervisor/Coordinator will perform in the capacity of coach for assigned Social Care Network staff providing guidance, interventions as needed and written documentation of such interventions within a timely fashion as outlined within the confines of the care management standards of care. The Care Coordination Supervisor will function in the role of representative for the Samaritan Daytop Village Inc. care management component, Hope Care Management. The SCN Supervisor/Coordinator will demonstrate a standard of excellence in all manners of agency business. The SCN Supervisor/Coordinator will have a substantial knowledge of medical and social determinants of care that may impact the focused population.

Responsibilities


What You Will Do


 

  • Performs oversight of assigned Social Care Network staff by providing guidance, interventions as
    needed and prepares written documentation of such interventions within a timely fashion.
  • Handles disciplinary actions, corrective action plans, performance improvement plans and staff
    evaluations.
  • Performs timely reviews of completed Assessments, Semi-Annual Evaluations and Care Plans.
  • Handles individual and group supervision.
  • Conducts weekly- biweekly meetings or as needed with unit members to discuss client issues,
    provide crisis interventions and/or assistance to care units as needed.
  • Assesses skill level of staff and develops individualized training plans or recommends
    appropriate internal/external workshops and trainings to increase staff development.
  • Conducts quality assurance audits and activities to assess effectiveness of services
    delivered, to ensure member charts and documentation are in compliance with Social Care Network polices and to certify that quality services are delivered to members.
  • Participation in external Health Home Lead meetings as needed and/or requested
  • Ensures care management team provides care coordination, appropriate linkages, referrals and
    follow-ups for needed services and supports as well as addresses barriers to health care, including
    but not limited to financial management, housing, transportation, social support and understanding of health conditions.
  • Assist with intakes as is needed.

Qualifications


Who You Will Be


  • Master’s Degree in Human Services or one of the qualifying fields such as child and family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation, recreational therapy, rehabilitation, social work, sociology, or speech and hearing and Two (2) years related experience; providing direct services to people with serious mental illness, developmental disabilities or substance use disorders or in linking individuals with serious mental illness, developmental disabilities or substance use disorders to a broad range of services essential to successful living in a community setting (i.e. medical, psychiatric, social, educational, legal, housing and financial services).
    **A Master’s degree in a related field may substitute for up to one year of experience**
    OR
  • Bachelor's Degree in Human Services or related fields such as child and family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation, recreational therapy, rehabilitation, social work, sociology, or speech and hearing with Three (3) years related experience as a Health Homes Care Manager/Patient Care Navigator;
    OR
  • An OASAS Credentialed Alcoholism and Substance Abuse Counselor (CASAC) with Three (3) years of related experience.
  • At least Two (2) years’ experience supervising clinicians and/or Care Managers who are providing direct services to individuals with Serious Mental Illness (SMI)/serious Substance Use Disorders.
  • Flexibility is needed as members may call outside of daily work schedule (24-hour call).
  • Willingness to travel to members’ homes as needed.
  • Ability to complete mandated training on the New York State Community Mental Health Assessment Instrument and additional required training.
  • Computer literacy including proficiency in Microsoft Office Suite and EHR.
  • Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.
  • Competency in written, interpersonal, verbal and computational skills to present and document records in accordance with program standards.
