The Role


Under the general supervision of the RN and /or Physician the Health Care Coordinator will support providers in the provision of health care including recording vital signs, discussing chief health concerns, performing testing, and assisting with procedures and documentation for designated approved OASAS programs. Assesses, plans, evaluates, and delivers patient care in collaboration with RN and/or Physician. Additionally, he/she is a member of the interdisciplinary clinical team at their assigned facility and participates in case conferences, treatment, and discharge planning meetings.  The incumbent will serve as a liaison between the agency’s clinical treatment team and the external primary medical care provider ensuring continuity of care for clients with medical conditions. This work is carried out in support of the mission and goals of Samaritan Daytop Village.

Responsibilities


What You Will Do


  • Assists RN or Physician with patient care including checking vital signs, blood pressure, pulse rate, breathing, temperature, and weight, discussing chief health concerns, and performing tests.
  • Takes patient’s medical history, all pretreatment formalities, and enters data in the computer system.
  • Follows orders of physician and registered nurse in providing care.
  • Ensures that medications are given to the intended patient in a safe and correct manner with proper documentation.
  • Assists self-directed patients in taking medications.
  • Handles proper charting, documentation, and maintenance of medical records.
  • Records patient clinical records are a detailed report of care given.
  • Reinforces instructions given to patients by RN or Physician.
  • Notifies RN or Physician of significant findings or changes in patient’s physical or mental condition.

Qualifications


Who You Will Be


 

  • BLS certification.
  • High School Diploma or Equivalent.
  • At least One (1) year experience working in a human services program in a caregiving role.
  • Available to work a flexible schedule as needed in response to participant and staff needs.
  • Computer literacy including proficiency in Microsoft Office Suite and EHR.
  • Ability to put patients at ease and explain physician’s instructions.
  • Ability to multi-task with excellent organizational skills.
  • Ability to maintain the confidentiality of patient records.
  • Experience working directly with people from diverse racial, ethnic, and socioeconomic. backgrounds.
  • Knowledge of medical terminology.
  • Team player and ability to work independently.
