The Role


The Outreach worker is essential in locating, engaging and enrolling assigned Health Home eligible persons who have been identified by the NYS Department of Health to be in need of comprehensive and coordinated social services. Outreach activities are defined within reason as electronic searches, correspondence, telephone calls, home visits, field visits and/or agency visits. Transportation is provided for all outreach activities or is reimbursed within the parameters of participating Health Home providers. This work is carried out in support of the mission and goals of Samaritan Daytop Village.

Responsibilities


What You Will Do


  • Perform community-based outreach activities such as home visits, provider visits, telephone calls and correspondence.
  • Confirm patient information and coverage via EPACES.
  • Follow engagement activities from an assigned calendar provided monthly.
  • Document all patient outreach activities in the Health Home EMR (electronic medical record), complete a Time Log reflecting the outreach activity and submit to Intake Coordinator daily.
  • If an Intake is agreed upon with potential enrollee complete the Demographics Form, A FACT GP, and the correct Health Home releases form. Upon return to the office complete a Time Log and a Progress Note in the Health Home EMR.
  • Prepare and maintain Health Home intake packets for each individual Health Home.
  • Respond to calls from Health Home regarding patients at noted hospital Emergency Rooms.
  • Complete Intakes as assigned.
  • Complete Bottom-Up Screening Forms as needed.
  • Any tasks or duties assigned by Managing Staff and/or Program Director.

Qualifications


Who You Will Be


  • High School Diploma or GED
  • At least One (1) years’ work experience or volunteer experience in a recovery-oriented setting.
  • Certified Recovery Peer Advocate-CRPA or CRPA-Provisional for designated OASAS programs
    • OR
  • New York Certified Peer Specialist -NYCPS or NYCPS for designated OMH programs.
  • Knowledge of substance use disorder recovery process.
  • Computer literacy including familiarity in Microsoft Office Suite.
  • Must be available to travel throughout the required catchment area as needed for applicable programs and able to navigate the public transportation systems to client’s home and within the community.
  • Commitment to recovery-oriented systems of care.
  • Demonstrate ability to work independently.
  • Excellent time management.
  • Ability to think logically, prioritize and follow procedures and instructions.
